When Fear Changes the Rules of Competition
Imagine that someone you love is rushed to an emergency room. The doctor says treatment must begin now. Do you ask whether another hospital is cheaper? Do you wait for a sale? Do you walk away?
For most families, the answer is no. When life may be at stake, saving someone we love overwhelms the bargaining habits we use in an ordinary marketplace.
The Customer Who Cannot Walk Away
Capitalism works especially well when customers can compare prices, choose another seller, postpone a purchase or simply say no. Serious healthcare can remove that power. Economists call this inelastic demand: even when price rises, the need for essential treatment may change very little.
Fear Meets Trust
Medicine has long recognized the vulnerability of the sick. The ethical tradition associated with Hippocrates places the patient's welfare at the center of medical practice and emphasizes avoiding harm. The familiar phrase "first, do no harm" summarizes that tradition, although those exact words are not in the original Hippocratic Oath.
That is why this series separates medicine from the financial system surrounding medicine. Doctors, nurses and caregivers deserve respect. Hospitals, insurers, contracts, prices, accounting rules and government programs create a separate economic structure that deserves careful examination.
Identify the Problem Before Prescribing the Cure
America did not create today's healthcare system in one decision. Prospective payment, HMOs, employer insurance, government programs, negotiated prices, networks and accounting practices developed over decades. Many were created to solve real problems. But policies can interact in ways their designers did not anticipate.
This series will proceed like a diagnosis: What is happening? How did it develop? Which practices are lawful? Which deserve scrutiny? Which rules are misunderstood or weakly enforced? What can America learn from other democracies?
A market works best when the customer can say no. A patient fighting for life often cannot.
NEXT IN THE SERIES: Medicine Is More Than a Business -- How do we protect medical independence from the financial pressures surrounding treatment?
Editor's note: This series distinguishes documented facts and established law from allegations, disputed interpretations and the author's proposed legal, tax and economic theories.

The latest US-China summit covered a wide range of topics including artificial intelligence (AI). However, the official readouts from both countries did not touch upon an issue that has generated heated debate: whether the United States and China should agree to pause or slow down the development of frontier AI. That absence has triggered considerable discussion. During my recent visit to New York, many US friends asked whether Beijing would ever agree to some form of arrangement. Before...

In August, US employment in roles most exposed to AI for people aged between 22 and 25 shrank by 4.4 percent year-on-year. This means there's been a contraction every single month since October 2023, about a year after ChatGPT was first released. The high impact of AI on younger workers is one of the key trends emerging from the Canaries Dashboard, a project led by the Stanford Digital Economy Lab and ADP Research. We spoke to ADP Research's Chief Economist Nela Richardson in our Business segment.
On September 22, the global artificial intelligence landscape spun on its axis within a matter of hours. In Beijing, Xiaomi engineers went live with an unusual broadcast, streaming the training run of their new MiMo-V2.6 model. In San Francisco, Anthropic unveiled its frontier Opus 5.5 model. About an hour later, OpenAI crashed the party with the surprise release of its GPT-6 Sol and Luna models. For tech executives and developers trying to keep pace, it was just another overwhelming day in the...
A malicious spreadsheet can make LibreOffice and Apache OpenOffice run an attacker's code as soon as the file is opened, security researchers have shown. There is no warning first, of the kind either program shows before it runs a macro. The attack works only when the program's Java support is enabled. So far, it has only been shown as a proof of concept, and there are no reports of its use in
